I believe its original to the 1976 Chevy, 1 ton, 454, manual trans truck I took it off of.
-Is it a good base for a high performance street car: 355 chevy, 400 h.p., auto trans?
-Is it considered the 4M or the M4M?

 Yes, that is the correct Qjet off your 1976 Chevrolet 1-Ton 454cid Truck.

 As for a good base, yes, I think it is. Just need to provide more info for Cliff. Like cam specs, rear gear ratio, tire size, etc.

 Also, it is a 4MV carb.

 Not sure I understand your question? Your part number is 17056212 with a date code 2166 (or 216th day of 1976).
